How the questions are built
1. Written against public outlines
Each question targets one domain of the published nurse aide exam outline: safety, infection control, personal care, communication, rights, data collection, basic nursing, mental health, nutrition, elimination and the role of the aide. The domain is shown with every question.
2. One best answer, four options
Every question has four options and one answer that follows the aide's scope of practice. When two options could both happen in real life, the correct one is the action within the aide's role: report, protect, or ask the nurse. That is the rule the real exam applies.
3. Every answer has a reason
No question ships without a one or two sentence explanation of why the answer is right. If the reason is not obvious to a reader who has never worked in a facility, the question is rewritten.
4. Correction loop
Every question carries a Report a mistake link. Reported questions are reviewed and either corrected or removed. There is no penalty for reporting, and no argument.
5. What the readiness score means
The score is the percentage of the 25 questions you answered correctly. The pass zone marker sits at 80% because most states require 70 to 80% on the written exam. It is an estimate from 25 questions, not a prediction. A 76% score means "close, keep going", not "you will fail".
6. What is not covered
The clinical skills demonstration, your state's application process, background checks and the exact scoring rules of your state. Those depend on your state board, and we link to the official sources in Sources.
